Waiver, Release, Indemnification
I wish to participate in an event hosted by Rocky Mountain Entertainment Agency INC. DBA The Rocky Mountain Agency. I expressly assume any and all risks of injury arising from or reliant to the activities and waive any release of any and all actions, claims, suites or demands of any kind or nature whatsoever against The Rocky Mountain Agency, it’s corporate affiliates, contractors, vendors, officer, agents, sponsors, volunteers or Representative of any kind (collectively “Releases”) arising from or relating in any way to my voluntary participation in these activities.
Cancellation Policy
The Rocky Mountain Agency retains the right to reschedule and/or change the events if needed. Refunds will be up to the discretion of The Rocky Mountain Agency. In the event that you cancel moore than 48 hrs prior to an event you will be charged a 50% cancellation fee and the remaining 50% will be credited to a future event and must be redeemed within 12 months of funds credited. If cancelled within 48 hours of the beginning of an event you will forfeit any remaining credit balance you would have had and will receive no refund. Any and all outstanding balances remaining will be collected by talent jobs that are done within the payment agreement term.
Lost or Stolen Items
The Rocky Mountain Agency is not responsible for any personal items that are lost, damaged, or stolen while attending events.
Photo Release
All photographs and video taken may be used to the sole discretion of The Rocky Mountain Agency for any advertising and promotional use. The Rocky Mountain Agency retains the right to edit and enhance all photos and videos without the consent of the participant.
Consent Clause
I have read and understand this waiver, release, and indemnification agreement, and consent on behalf of the participant to its terms.
COVID-19 Waiver and Consent
In exchange for being permitted to enter the premises of a The Rocky Mountain Agency event I agree to the following as described in this contract.
Customer agrees to instructions and requirements:
I will follow all of the instructions of the Business while on the Premises, including: business guidelines and requirements
I agree not to enter the Premises if I am experiencing symptoms of COVID-19 such as cough, shortness of breath, or fever, have a confirmed or suspected case of COVID-19, or have come in contact in the last 14 days with a person who has been confirmed or suspected of having COVID-19.
Customer assumes the risk
I am aware of the highly contagious nature of COVID-19 and the risk that I may be exposed to or contract COVID-19 by being on the Premises and engaging in the Activity.
I acknowledge that I am voluntarily entering the Premises to engage in the Activity with knowledge of the danger involved. I hereby agree to accept and assume all risks of personal injury, illness, disability, or death related to COVID-19, arising from my being on the Premises or engaging in the Activity, whether caused by negligence of the Business or otherwise.
Customer releases seller from liability
I hereby expressly waive and release any and all claims, now known or hereafter known, against the Business and its owners, employees, affiliates, and officers, on account of injury, illness, disability, or death arising out of or attributable to my being on the Premises or engaging in the Activity and being exposed to or contracting COVID-19, whether arising out of the negligence of the Business, its owners, employees, affiliates, officers, or otherwise.
